In a separate bowl, mix grated parmesan, panko bread crumbs, and melted butter until evenly combined.
Sprinkle the bread crumb mixture over the dip in the Crock Pot, covering it completely.
Cover and cook on LOW until hot and bubbly, stirring halfway through.
